Safety Precautions
We routinely screen patients, visitors, and staff for signs and symptoms of respiratory infection. Masks are required to be worn at all times by patients, visitors, and staff when in the office. Masks must cover the nose and mouth.
Visitor Policy
Each patient is permitted to have one (1) adult visitor to accompany them for their appointment. No visitors under the age of 18 years are permitted.
Cancellation and Late Arrival Policy
We understand that your plans may change. If you need to cancel or reschedule your appointment, we ask that you do so at least 24 hours in advance. Cancellations made less than 24 hours in advance will incur a no-show fee. If you do not show up for your appointment or are more than 15 minutes late, you will incur a no-show fee.
Payment
We accept payment in the forms of cash, credit card, debit card, or HSA card. Personal checks are not accepted. It is office policy to keep a credit, debit, or HSA card on file for every patient.
